These were built by veterans like us, for veterans like us, and with our needs and concerns in mind. They address the needs and concerns of ALL veterans regardless of who they are or who they love. Some are specifically tailored for certain populations of veterans particularly when the suicide rates and suicide ideation is much higher for some of those populations than others. They were built in collaboration with many individual and organizational teams from beginning to end.

The VACCN questionnaire

VA Community Care Network and Prescriptions questionnaire - for ALL veterans.

This questionnaire was conceptualized and developed by TAVA Board Member Stacy Lunsford in collaboration with Cassandra Williamson, TAVA’s Director of Research & VA Engagement and with Jennifer Hunt our amazing VSO liaison in the office of Under Secretary of Health in VHA. Stacy developed the questions, Jennifer made some valuable suggestions for improving the questions, and Cassandra put their work into the REDCap survey build system.
Thanks you for completing the questionnaire. This questionnaire will remain active until August 31st, 2022 at which time the de-identified data will be reviewed and submitted to VA national, the house and senate VA committees, the White House, and other interested parties. The data will remain be retained and owned by TAVA.

This is a quality of service (quality improvement) reporting system for veterans and military personnel who also identify in the LGBTQ+ community.

This QA reporting tool (QSRS) has been developed by a remarkable team in the Transgender American Veterans Association (TAVA) as well as by members from many other trans/nb/gender diverse and LGBQ+ veteran/military organizations and groups. It is broadly available to ALL LGBTQ+ veterans and active-duty military.

For follow-up on issues and concerns raised, IT DOES GATHER PERSONAL INFORMATION.

HOWEVER, when we issue summaries of the information gathered, your Personally Identifiable Information WILL be removed.

Please also use this system to compliment those providers who are providing exceptional service to you.

Finally, you are encouraged to use this system as often as you want. Share it with others as well.

This survey will end the last day of August 2022. It is designed to gather data about the health needs and concerns of transgender/non-binary/gender-diverse/gender-expansive veterans. It was developed by veterans who identify under the transgender umbrella of identities, for this population, and Institutional Review Board approved by the University of Alabama at Tuscaloosa. This is sound academic survey. It is very in-depth and has break points built in so that a veteran can come to back to it when they desire.
